รีวิวเคสของคลินิกทันตกรรมเดอะคราวน์โคราช คุณหมอรับย้ายเคสของคนไข้ต่างชาติมา โดยคนไข้ถอนฟันหน้า 4 ซี่ มีฟันปลอมติดกับแบ็คเก็ตมาด้วย จากการตรวจช่องปาก พบว่า ฟันเขี้ยวบนซ้ายและขวายื่น ฟันกรามน้อยไม่สบกับฟันล่าง ตำแหน่งฟันเขี้ยวซ้ายและขวาไม่อยู่ในตำแหน่งปกติ

เคสนี้ต้องแก้ไขฟันเขี้ยวที่ยื่น เพื่อให้การใส่ฟันปลอมบริเวณฟันหน้าไม่ยื่นจนเกินไป ซึ่งจะดูไม่สวยงาม

ซี่ฟันปลอมที่ยึดติดกับแบ็คเก็ตในระหว่างการจัดฟัน

ภายหลังการจัดฟันเสร็จสิ้น คุณหมอทำการทดแทนฟันหน้าด้วยงานฟันปลอมถอดได้ชนิดฐานนิ่ม หรือวอลพลาสต์ ร่วมกับเครื่องมือคงสภาพฟัน (Retainers) ที่มีซี่ฟันปลอมในชิ้นงานด้วย โดยแนะนำให้คนไข้ใส่สลับกัน โดยช่วงกลางวันแนะให้คนไข้ใช้งานฟันปลอมบน ร่วมกับรีเทนเนอร์ชิ้นล่าง. ส่วนช่วงเวลานอน ให้ใส่รีเทนเนอร์บนและล่าง เคสนี้แนะนำให้ใส่รีเทนเนอร์ประมาณ 1 ปี และนัดกลับมาเช็คทุก 6 เดือน ตามระยะ

ฟันเขี้ยวด้านซ้ายและด้านขวาได้ถูกจัดเข้าตำแหน่งสบฟันที่เหมาะสม ส่งผลให้งานฟันปลอมสามารถจัดเรียงมุมฟันได้สวยงาม ไม่ยื่นจนเกินไป 

Case Review from The Crown Dental Clinic, KoratIn this case, the dentist accepted a transferred orthodontic case from an international patient. The patient had previously had four upper front teeth extracted and was wearing provisional denture teeth attached to orthodontic brackets.Upon oral examination, it was found that both the upper left and right canines were protrusive, the premolars did not occlude properly with the lower teeth, and the positions of the left and right canines were not in their normal anatomical alignment.This case required correction of the protruding canines so that the replacement of the anterior teeth would not appear excessively protrusive, which would compromise esthetics.Provisional denture teeth attached to orthodontic brackets during orthodontic treatmentAfter completion of orthodontic treatment, the dentist replaced the anterior teeth with a removable flexible denture (soft-base denture / Valplast), used in combination with retainers that incorporated artificial teeth within the appliance. The patient was advised to alternate their use as follows:

• During the daytime, the patient should wear the upper removable denture together with the lower retainer.
• During sleep, the patient should wear both the upper and lower retainers.

For this case, the patient was advised to wear the retainers for approximately one year and to return for follow-up examinations every six months according to the recommended schedule.
