ตัวอย่างเคส คนไข้เคยบูรณะสะพานฟันด้านซ้ายและขวา พบว่ามีเหงือกบวมบริเวณด้านซ้าย และต้องการเปลี่ยนสีฟันให้ขาวขึ้น จึงมีความจำเป็นที่ต้องแก้ไขเหงือกที่บวมด้วยการศัลกรรมเหงือกให้เรียบร้อยก่อน จากนั้นรื้อสะพานฟันเดิม ร่วมกับทำครอบชั่วคราว ในตำแหน่งเหงือกใหม่ ให้เรียบร้อย แล้วจึงบูรณะฟันหน้าทั้ง 4 ซี่ ด้วยวีเนียร์เซรามิกต่อไป

เนื่องด้วยฟันหลังเป็นการบูรณะสะพานฟันทั้งซ้ายและขวา คุณหมอต้องแนะนำเรื่องของการทำความสะอาดบริเวณซอกฟันอย่างเคร่งครัดให้กับคนไข้ เช่นการใช้ไหมขัดฟันชนิดแข็ง (Super Floss) สอดทำความสะอาดใต้สะพานฟัน และการกลับมาเช็คสภาพฟันตามระยะทุก 6 เดือน เพื่อคงสภาพชิ้นงานและสภาพเหงือกที่ดีในระยะยาวต่อไป

Case Example (English Translation)This case involves a patient who previously received fixed bridge restorations on both the left and right sides. Clinical examination revealed gingival swelling on the left side, and the patient also expressed a desire for a brighter tooth color. Therefore, it was necessary to first correct the inflamed gingiva through periodontal surgery. After the gingival condition was stabilized, the existing bridges were removed and provisional crowns were fabricated to accommodate the new gingival contours. Subsequently, ceramic veneers were placed on the four maxillary anterior teeth.Because the posterior teeth were restored with fixed bridges on both sides, the dentist emphasized the importance of meticulous oral hygiene in the interdental areas. The patient was instructed to use stiff dental floss (Super Floss) to clean beneath the bridge pontics, as well as to return for routine follow-up examinations every six months to maintain both the restorations and long-term gingival health.In contrast, the anterior veneer restorations were individual units for each tooth, making daily oral hygiene easier. Cleaning can be effectively maintained with regular dental floss, followed by routine toothbrushing.In this case, when the patient produced an extremely wide smile or during normal conversation, it was observed that the lower lip covered most of the mandibular teeth, making the lower tooth color minimally visible to others. Therefore, treatment of the maxillary teeth alone was considered sufficient. The patient agreed with this approach and reported a high level of satisfaction with both the color and shape of the veneers achieved in this treatment.
