เมื่อคุณอาสูงวัย อยากมีฟันสวยไว้ใช้งานบ้าง!?

รีวิวเคสคนไข้ของคลินิกทันตกรรมเดอะคราวน์ โคราช คนไข้ต้องการทำครอบฟันหน้าที่ทำค้างไว้ แก้ไขฟันหน้าล่างที่สั้น และใส่ฟันปลอมด้านหลัง 

จากการตรวจพบว่า ไม่มีพื้นที่ในการบูรณะฟันหลัง ฟันหลังด้านซ้ายไม่มี และฟันหลังด้านขวาฟันสึก คุณหมอวางแผนในเฟสแรก ทำฟันปลอมถอดได้ชุดแรกที่ระดับความสูงใหม่ ให้คนไข้ใส่ใช้งาน เพื่อประเมินว่าคนไข้สามารถปรับตัวที่ระดับความสูงที่กำหนดใหม่ ได้หรือไม่ โดยใส่ประมาณ 3 เดือน

หลังจาก 3 เดือน คนไข้ใช้งานฟันปลอมชุดแรกได้ OK ดี ไม่มีปัญหาข้อต่อขากรรไกร ใส่เคี้ยวอาหารได้ตามปกติ ก็จะเข้าสู่การทำงานฟันปลอมติดแน่น คือการบูรณะส่วนของสะพานฟัน (ด้านขวา) ครอบฟันหน้าล่าง และฟันปลอมแบบถอดได้(ด้านซ้าย) ที่ระดับความสูงใหม่  โดยคุณหมอทำเป็นงานสะพานฟัน และครอบชั่วคราว ให้คนไข้ใส่ใช้งาน

เปลี่ยนงานครอบชั่วคราว เป็นงานครอบฟัน สะพานฟันชุดสุดท้าย ช่องถอนฟันที่เหลือคนไข้เลือกเป็นงานฟันปลอมแบบถอดได้ (แทนงานรากเทียม)

ณ ปัจจุบัน นัด Recall ทุก 1 ปี เข้าปีที่ 6 แล้ว มีการเปลี่ยนชุดฟันปลอมถอดได้บนล่าง เจอฟันผุที่คอมพอสิตวีเนียร์ฟันหน้า แต่ครอบฟันที่ทำยังมีสภาพที่ดี ไม่โยก เหงือกไม่ร่น แนะนำเปลี่ยนฟันปลอมถอดได้ เป็นรากฟันเทียมในอนาคต 

Patient Case Review – The Crown Dental Clinic, KoratThe patient visited the clinic seeking completion of previously unfinished anterior crown work, correction of short lower front teeth, and replacement of missing posterior teeth.Clinical examination revealed insufficient space for posterior restoration. The posterior teeth on the left side were missing, while the posterior teeth on the right side showed significant wear. In the first phase of treatment, the dentist planned to fabricate an initial removable denture at a newly established vertical dimension. The patient was instructed to wear this appliance for approximately three months to evaluate adaptation to the new vertical height.After three months, the patient adapted well to the initial removable denture. There were no temporomandibular joint (TMJ) problems, and the patient was able to chew normally. Treatment then progressed to fixed prosthodontic rehabilitation, including a fixed bridge on the right posterior side, crowns on the lower anterior teeth, and a removable denture on the left posterior side—all at the new vertical dimension. Temporary bridges and temporary crowns were fabricated and provided for functional use during this phase.Subsequently, the temporary restorations were replaced with the final crowns and fixed bridge. For the remaining extraction spaces, the patient chose a removable denture instead of dental implants.At present, the patient is recalled annually and is now in the sixth year of follow-up. The upper and lower removable dentures have been replaced once. Caries was detected in the composite veneer restorations on the anterior teeth; however, the crowns remain in good condition, stable, with healthy gingival tissues and no recession. The patient has been advised to consider replacing the removable dentures with dental implants in the future.
