เดือยฟัน คืออะไร? บูรณะไปเพื่ออะไร จำเป็นไหม?
ฟันที่ผ่านการรักษารากฟันมา โดยเฉพาะฟันหลัง ซึ่งต้องใช้ในการรับแรงจากการบดเคี้ยวอาหาร ซึ่งเป็นแรงที่สูงมากที่จะกระทำกับฟัน ฟันที่ผ่านการรักษารากฟันมา ค่อนข้างบอบบางและอ่อนแอ การรับแรงกัดที่สูงมากๆ อาจก่อให้เกิดความเสียหายได้
ดังนั้นหลังการรักษารากฟัน คุณหมอจะแนะนำให้ท่าน ทำเดือยและครอบฟันต่อ ซึ่งจะช่วยเพิ่มความแข็งแรงต่อฟันซี่ได้มากขึ้น และช่วยยืดอายุการใช้งานของฟันซี่ดังกล่าวได้มากขึ้นด้วย
“เดือยฟัน” คือวัสดุที่มีลักษณะเป็นแท่งยาวๆ ใช้ปักลงไปในรากฟัน หลังจากอุดคลองรากฟันแล้ว โดยจะทำการเจาะวัสดุอุดคลองรากฟันลงไปประมาณ 2 ใน 3 ของความยาวรากฟัน หลังจากนั้นจึงทำการยึดเดือยฟันด้วยซีเมนต์โดยจะมีส่วนหนึ่งของเดือยฟันโผล่ขึ้นมาจากส่วนของรากฟัน เพื่อมาเป็นส่วนแกนรองรับส่วนของครอบฟันอีกที เพื่อสร้างความแข็งแรงให้กับฟันซี่นี้
จากภาพ เป็นลักษณะของเดือยฟันโลหะ (Metal Post and Core) ประกอบไปด้วย ส่วนที่ใส่เข้าไปในรากฟัน และส่วนที่โผล่ขึ้นมาเหนือรากฟัน ที่จะทำหน้าที่เป็นแกนฟัน รองรับส่วนครอบฟัน
เดือยฟัน มีหลายชนิด แต่คุณหมอแบ่งแบบเข้าใจง่ายๆ ประมาณ 2 ชนิดหลัก คือ
1. เดือยฟันที่ต้องสแกนช่องปากแล้วส่งไปขึ้นชิ้นงานกับช่างทันตกรรม หรือทำกับเครื่องกลึงนอกปาก แล้วค่อยนัดคนไข้กลับมาใส่อีกครั้ง มักเลือกทำ กรณีที่เนื้อฟันจริง ของฟันซี่นั้นเหลือไม่มาก หรือต้องการความแนบสูงๆ
2. เดือยฟันสำเร็จรูป ก็จะเป็นชิ้นสำเร็จรูปมา ใส่ในปากได้เลย ส่วนมากจะเลือกทำในฟันที่เหลือเนื้อฟันจริง ค่อนข้างมาก ใช้ร่วมกับวัสดุอุดฟัน เพื่อสรา้งส่วนของแกนฟันขึ้นมา

Post and Crown Restoration After Root Canal Treatment – The Crown Dental Clinic, KoratTeeth that have undergone root canal treatment—especially posterior teeth that must withstand strong chewing forces—are generally more fragile and prone to fracture. Because these teeth no longer have vital pulp, their ability to absorb high biting forces is reduced, which may lead to structural damage over time.For this reason, dentists typically recommend placing a post and crown after root canal treatment. This approach significantly strengthens the tooth and helps extend its functional lifespan.A dental post is a long, rod-like material placed into the root canal after completion of root canal filling. Approximately two-thirds of the root canal filling material is removed, and the post is cemented into the canal. Part of the post extends above the root to serve as a core, supporting the final crown and reinforcing the tooth structure.The image shown illustrates a metal post and core, consisting of the portion inserted into the root canal and the portion extending above the root to support the crown.Dental posts can be broadly classified into two main types:
- Custom-made posts, which require intraoral scanning or impressions and fabrication by a dental laboratory or milling machine. These are commonly used when very little natural tooth structure remains or when high precision and fit are required.
- Prefabricated posts, which are ready-made and placed directly in the mouth. These are usually selected when sufficient natural tooth structure remains and are combined with restorative materials to build up the core.
Case example – The Crown Dental Clinic, Korat:
The patient underwent root canal treatment on an upper premolar and required definitive restoration. After removing the previous temporary restoration, the dentist found that very little natural tooth structure remained. Periodontal surgery was considered to increase clinical crown length, followed by placement of a metal post cemented into the root canal. The tooth was then restored with a crown.In this case, the final restoration was designed as a porcelain-fused-to-metal crown, providing both strength and durability while maintaining acceptable aesthetics.
