ตามปกติ ขั้นตอนในการทำฟันปลอม จะมีขั้นตอนหลักอยู่ประมาณ 5-6 ขั้นตอน วันนี้คุณหมอจะพามารู้จักขั้นตอน ก่อนที่ท่านจะได้ใส่ชิ้นงานฟันปลอม ว่ามีขั้นตอนอะไรกันบ้าง เพื่อเป็นการเตรียมความพร้อมก่อนเข้าสู่การรักษา

ขั้นตอนก่อนการเข้าสู่กระบวนการทำฟันปลอม 

เป็นขั้นตอนในการเตรียมสภาพช่องปากของคนไข้ เริ่มจากการตรวจและประเมินสภาพช่องปากก่อนการทำฟันปลอม คุณหมอจะตรวจตั้งแต่สุขภาพร่างกาย เช็คสัญญาณชีพ ซักประวัติโรคประจำตัวต่างๆ จากนั้นทำการตรวจสภาพช่องปาก ตรวจฟันและสันเหงือกที่ต้องใส่ฟัน 

ฟันที่ไม่สามารถเก็บไว้เป็นฟันหลักสำหรับใช้ยึดเกาะตะขอของฟันปลอมถอดได้ จะถูกวางแผนให้ถอนฟัน ส่วนฟันที่สามารถเก็บไว้ใช้งานต่อได้ จะได้รับการวางแผนให้อุดฟัน และขูดหินปูน เกลารากฟัน

การตรวจสันเหงือกที่จะใส่ฟัน เพื่อประเมินความกว้างของช่องที่จะใส่ฟัน ว่าเพียงพอในการบูรณะหรือไม่ มีกระดูกอูม หรือโผล่ออกมาซึ่งจะรบกวนการใส่ฟันในอนาคตหรือไม่ จำเป็นต้องผ่าตัด กรอแต่งกระดูกก่อนการทำฟันปลอมหรือไม่

ขั้นตอนการทำฟันปลอมถอดได้

2. ขั้นตอนการลอกเลียนเนื้อเยื่อในช่องปาก และการพิมพ์ปากครั้งสุดท้าย

เป็นขั้นตอนของการใช้วัสดุคล้ายดินน้ำมัน นำมาลอกเลียนขอบของสันเหงือกและเนื้อเยื่อ เพื่อกำหนดขอบเขตของฐานฟันปลอมว่าสามารถขยายไปได้ ณ จำแหน่งใด โดยไม่รบกวนการทำงานของเนื้อเยื่อ 

3. ขั้นตอนการลองแท่นกัดขี้ผึ้ง เพื่อหาตำแหน่งความสูงของใบหน้า ตำแหน่งที่คนไข้ใช้กัด (จุด Stop ของการกัด ที่ทำให้ข้อต่อขากรรไกรสบาย ไม่ปวด) การกำหนดตำแหน่งของฟัน ขนาดของฟัน และเช็คความอูมของส่วนรองรับเนื้อเยื่อต่างๆ

4. ขั้นตอนการเรียงฟัน และนัดคนไข้กลับมาลองฟันปลอม ก่อนที่จะใส่ชิ้นงานจริงในครั้งต่อไป เพื่อคอนเฟิร์มตำแหน่งที่ช่างทันตกรรมเรียงฟันมา ว่าเหมือนกับในครั้งก่อน ที่คุณหมอกำหนดตำแหน่งการสบฟัน และตำแหน่งการเรียงตัวของฟัน หรือไม่

5. ขั้นตอนการใส่ฟัน เช็คการสบฟัน การให้คำแนะนำหลังการใส่ฟัน ได้แก่

- คำแนะนำเรื่องการทำความสะอาดฟันปลอม การถอดฟันปลอมในตอนกลางคืน และการแช่น้ำไว้

- การฝึกใช้งาน การปรับตัวต่อการใช้งานฟันปลอม การให้กำลังใจ โดยการฝึกให้ฝึกใช้งานต่อเนื่อง วันละอย่างน้อย 4 ชั่วโมง หรือมากกว่า

- หากเกิดการบาดเจ็บจากการใส่ฟันปลอม ให้โทรกลับมานัดหมายเพื่อรับการแก้ไข จนกว่าจะใช้งานได้ปกติ (ไม่ขัดแต่งด้วยตนเอง)

- การติดตามอาการหลังการใส่ฟันตามระยะ และอายุการใช้งานของฟันปลอม การปฏิบัติตัวเมื่อฟันปลอมแตกหักเสียหาย

Pre-treatment preparation before denture fabrication

This initial stage focuses on preparing the patient’s oral condition. The dentist begins with a comprehensive examination, including general health assessment, vital signs, and review of medical history. An intraoral examination then follows, evaluating the teeth and gum ridges where the dentures will be placed.Teeth that cannot be preserved as abutments for removable dentures are planned for extraction. The first impression is taken to fabricate a custom impression tray, which will be used in the next stage.2. Border molding and final impression

A moldable material is used to record the contours of the gum ridges and surrounding soft tissues. This step defines the ideal extension of the denture base without interfering with normal tissue movement.3. Wax bite registration

This step determines facial height, the patient’s bite position, and a comfortable stopping point for jaw closure to ensure joint comfort. Tooth position, tooth size, and tissue support are also evaluated.4. Tooth setup try-in

The arranged teeth are tried in the patient’s mouth to confirm accuracy in bite position, tooth alignment, and aesthetics based on the previously established plan.5. Denture delivery and post-insertion care

At insertion, the dentist checks the bite and provides detailed instructions, including:

• Proper denture cleaning, removal at night, and soaking in water
• Training and encouragement for adaptation, recommending at least 4 hours of daily wear or more
• Instructions to return to the clinic for adjustments if sore spots occur (patients are advised not to adjust dentures themselves)
• Follow-up visits, denture lifespan guidance, and instructions on what to do if dentures are damaged

This structured approach ensures that dentures are comfortable, functional, and well-adapted to each patient’s oral condition, leading to long-term satisfaction and improved quality of life.
