รีวิวเคสจัดฟัน แก้ไขฟันล่างคร่อมฟันบน และฟันล่างยื่น (เคสคนไข้ประสบอุบัติเหตุ)

รีวิวเคสจัดฟันของคลินิกทันตกรรมเดอะคราวน์โคราช 

เคสนี้เป็นคนไข้ประสบอุบัติเหตุ มีการยึด Screw ที่ขากรรไกรล่าง จากการตรวจพบมุมแกนฟันหน้าล่างยื่น ร่วมกับมีกระดูกขากรรไกรยื่นร่วมด้วย

อธิบายแผนการรักษา แนะนำผ่าตัดขากรรไกร น่าจะเซฟฟันหน้าได้ ถ้าเลือกแก้ไขด้วยการจัดฟันอย่างเดียว มีโอกาสเสี่ยงต่อฟันหน้าล่าง ตัวฟันล้มเข้ามาทางด้านลิ้น ส่วนรากฟันหน้าล่างอาจจะเกิดการละลายของกระดูกเบ้าฟันได้ ซึ่งนำไปสู่การถอนฟันและทดแทนด้วยฟันปลอมแบบต่างๆตามมา คนไข้พิจารณาเลือกจัดฟัน ปฏิเสธการผ่าตัด  

คุณหมอส่งเคลียร์ช่องปาก อุดฟัน ขูดหินปูน รักษารากฟันหน้าล่างที่แตกหักและเปลี่ยนสี ร่วมกับอุดแต่งเป็นซี่ฟันด้วยวัสดุคอมพอสิตขึ้นมาในระหว่างการจัดฟันก่อน เคสนี้วางแผนปัก Screw เพิ่มในฟันล่าง เพื่อแก้ไขฟันบิดเก เนื่องจากฟันหลังถอนไปหลายซี่ 

คนไข้เลือกแบ็คเก็ตในระบบฟาสต์ ชนิดโลหะรัดยางสองชั้น หรือ AOSC เพื่อช่วยให้ฟันเคลื่อนตัวได้เร็วในช่วงของการไล่ระดับฟันและเรียงฟัน

ภายหลังการจัดฟันให้ฟันเข้าตำแหน่งที่เหมาะสม พบมุมแกนฟันหน้าล่างไม่ล้มเข้าด้านในมากเกินไป ถ่ายภาพเอ็กซ์เรย์ด้านข้างกะโหลกศีรษะ เพื่อเช็ครากฟันหน้าล่าง พบว่ายังอยู่ภายในเบ้ากระดูกดี ไม่พบการละลายตัว แต่ก็ต้องติดตามเป็นระยะ

ณ ปัจจุบันคนไข้ถอดเครื่องมือจัดฟัน และใส่เครื่องมือคงสภาพฟัน และกลับมาเช็คสุขภาพฟันตามระยะทุก 6 เดือน

Case Review – Orthodontic Treatment for Lower Incisor Protrusion after Mandibular Trauma – The Crown Dental Clinic, KoratThis patient had a history of a jaw trauma requiring a screw fixation in the lower jaw. On examination, the lower incisors were protruding with an associated mandibular protrusion.Treatment Planning & Considerations:

• Surgery was initially recommended to prevent damage to the lower incisors. Without surgery, there was a risk that the lower incisors could tilt lingually, causing root resorption and potentially leading to tooth loss. The patient opted for orthodontic treatment only, declining surgery.
• Preparatory work included clearing the oral cavity, dental restorations, scaling, and root canal treatment for fractured and discolored lower incisors. Composite buildups were placed on affected teeth during orthodontic treatment.
• Additional screws were planned in the lower jaw to correct rotated teeth, particularly due to multiple missing posterior teeth.
• The patient chose the AOSC fast metal dual-ligating bracket system to accelerate tooth alignment and leveling.

Outcome:

• After orthodontic treatment, the lower incisors were properly aligned, with minimal lingual tipping.
• Lateral cephalometric radiographs confirmed the roots of the lower incisors remained well-positioned within the alveolar bone, with no signs of resorption.
• The patient is now using retainers and attends follow-up appointments every 6 months to monitor stability and oral health.

This case highlights that careful planning and the use of adjunctive devices, like screws and dual-ligating brackets, can achieve successful orthodontic outcomes even in complex post-trauma cases while minimizing the risk of root resorption
